How to refer?
Referral Criteria:
- Usually 50+ years old
- Registered with MK GP’s or Living in MK Council Boundary
- History of fall
- identified at risk of fall or fear of falling
Who can refer?
- GP
- Any health and social care professional including Ambulance Services
- Self-referral
